There have been many posts about users having problems with their Ultra boxes. I had the same problems with my box, but after many trial-n-fail I have now a fully working box. I hope this guide helps those who need it.
NOTE: If your box is working do NOT follow these steps. Again, this worked for me, your box may have different issues and it may not work for everyone.

Before you start, make sure you download factory BIN v119 (for some reason v131 gave me a lot of problems) and the latest BIN fix.
-First thing I did was a Factory Reset to delete all the channels.
-I then loaded the Factory BIN v119
-After the box reboots, load the BIN fix and wait for the box to reboot again.
-As soon as the box turns on, do a hard reboot by turning the box off using the rear power switch.
-Wait about 30 secs and turn the box on (the box will read NO CHNLS)
-Go to MENU>ANTENNA SETTINGS and change the setting for the sats.

Here are the setting for 110, 119 and 91 (which are the ones I have, you might need to check the settings for any other bird)

119. ECHO 7
LNB Power: ON
LNB Type: Standard (For dish pro set to OCS)
LNB Freq: 11250
22kHz: Off (If you have a 22kHz switch, turn on)
TP Freq: 12339
Diseqc 1.0: Port 1
Motor: Off
Legacy SW: None

Press F1 to scan the sat TP. EXIT and save changes.

Go to next sat

110.0W Echostar 6,8
LNB Power: ON
LNB Type: Standard (For Dish pro set to OCS)
LNB Freq: 11250
22kHz: Off (If you have a 22kHz switch, turn on)
TP Freq: 12224
Diseqc 1.0: Port 2
Motor: Off
Legacy SW: None

Press F1 to scan the sat TP. EXIT and save changes.

Go to next sat

Nimiq 1,3 91.0W
LNB Power: ON
LNB Type: Standard (For dish Pro set to OCS)
LNB Freq: 11250
22kHz: OFF
TP Freq: 12224
Diseqc 1.0: Port 3
Motor: Off
Legacy SW: None

Press F1 to scan the sat TP. EXIT and save changes.

-Go to AUTO SCAN and scan each sat individually. Save all changes and EXIT.
You should now have all channels in working order. If for some reason a particualr channel is taking too long to show, just turn off the box from your remote and turn it back on. The channel should show now.
